Experienced Social Worker – Adults SPOA & Older People’s Team – Wrexham County Borough Council

Wrexham County Borough Council is seeking an experienced Social Worker to work within the Adult Social Work Teams, supporting the Single Point of Access (SPOA) and Older People’s Team. This is a great opportunity to make a difference to older adults and vulnerable individuals through person-centred social work practice.

Key Responsibilities:

  • Undertake comprehensive assessments and develop outcome-focused care and support plans for adults and older people.
  • Manage a varied caseload, prioritising effectively and working independently when required.
  • Prepare and present high-quality reports for case reviews, safeguarding meetings, and court proceedings.
  • Liaise with partner agencies, internal teams, and external organisations to deliver integrated support services.
  • Uphold the Council’s values and demonstrate professional practice in line with Social Care Wales’ Code of Professional Practice.

Essential Requirements:

  • Qualified Social Worker (QSW) with registration with Social Care Wales.
  • Strong experience working within adult social care teams, preferably in SPOA or older people’s services.
  • Excellent assessment, communication, and report-writing skills.
  • Proven ability to organise workload and work autonomously while maintaining strong relationships with partners.
  • Knowledge of safeguarding, care planning, and multi-agency working within adult services.

Additional Information:

  • Location: Wrexham Crown Buildings (hybrid working available).
  • Full-time: 37 hours per week (Monday to Friday, between 08:00 – 17:00).
  • Inside IR35.
