Job Description
Advanced Social Worker – Reablement Team (Adults) – London Borough of Islington
The London Borough of Islington is seeking an Advanced Social Worker to join its Reablement Team, delivering short-term interventions to support adults in regaining or maximising their independence. Working as part of a multidisciplinary team, you will guide service users with complex needs through the reablement process while promoting person-centred care.
Key Responsibilities:
- Manage a caseload of complex cases within the reablement service, coordinating multi-disciplinary input to meet individual goals.
- Carry out targeted assessments and reviews under the Care Act 2014 and Mental Capacity Act 2005.
- Lead on safeguarding enquiries, identifying and managing risk in line with Pan-London Safeguarding Adults procedures.
- Work collaboratively with therapists, pharmacists, home support workers, and case managers to maximise service user outcomes.
- Provide expert advice to internal colleagues and external partners (e.g., NHS, independent sector).
- Maintain accurate and timely records and contribute to case conferences, safeguarding meetings, and other forums.
Essential Requirements:
- Qualified Social Worker (QSW) with Social Work England registration.
- Minimum of 4 years post-qualified experience in adult social care, preferably within reablement or short-term services.
- Strong understanding of the Care Act 2014, MCA 2005, and safeguarding procedures.
- Excellent communication, assessment, and multi-agency working skills.
- Flexibility to work across a 7-day rota to meet service needs.
Additional Information:
- Location: 222 Upper Street, Islington.
- Full-time: 35 hours per week.
- Hybrid working arrangement.
- Inside IR35.
