Job Description

Senior Practitioner – Transitions & Progression to Adulthood Team – London Borough of Islington

The London Borough of Islington is recruiting a Senior Practitioner to join its specialist Transitions & Progression to Adulthood Team, supporting young adults with learning disabilities as they move into adulthood. This role combines case-holding responsibilities with supervisory duties, offering a balance of leadership and direct social work practice.

Key Responsibilities:

  • Manage a small caseload of complex cases involving young adults transitioning into adult services.
  • Provide supervision, mentoring, and support to approximately 4 social workers within the team.
  • Undertake statutory duties, including Care Act assessments, Mental Capacity Act assessments, safeguarding work, and care planning.
  • Promote a strengths-based, person-centred approach, enabling young people to achieve positive and sustainable outcomes.
  • Work collaboratively with families, carers, education, health, and adult services partners to ensure smooth transitions and progression planning.

Essential Requirements:

  • Qualified Social Worker (QSW) with Social Work England registration.
  • Minimum 3 years’ post-qualified experience, ideally with prior experience in a senior role or supervising staff.
  • Strong understanding of transitions, learning disabilities, and adult social care legislation.
  • Excellent assessment, leadership, and multi-agency working skills.

Additional Information:

  • Location: LBI Learning Disabilities Partnership, Islington.
  • Full-time: 35 hours per week.
  • Hybrid working model (minimum one office-based day per week, with flexibility for homeworking).
  • Inside IR35.
