Job Description

Transforming Care Social Worker – Adult Learning Disabilities – London Borough of Lambeth

The London Borough of Lambeth is looking for a skilled Social Worker to join its specialist Transforming Care Team within the Adult Learning Disabilities Service. This role focuses on preventing unnecessary inpatient admissions and supporting timely discharges into community settings for individuals with learning disabilities and/or autism.

Key Responsibilities:

  • Deliver robust, person-centred support planning to prevent inpatient admissions and ensure sustainable community placements.
  • Facilitate and support discharges from specialist inpatient units, collaborating with multi-disciplinary teams (MDTs) and community services.
  • Undertake Care Act assessments, safeguarding enquiries, and Mental Capacity Act assessments as required.
  • Lead on Best Interest decisions and contribute to care coordination for individuals at risk of hospital admission.
  • Participate in Transforming Care Panels, promoting community-based solutions in line with national and local objectives.

Essential Requirements:

  • Qualified Social Worker (QSW) registered with Social Work England.
  • Minimum of 2 years’ post-qualified experience, ideally within learning disabilities or autism services.
  • Strong working knowledge of the Transforming Care agenda, Care Act 2014, and safeguarding principles.
  • Proven experience in MDT working, hospital avoidance, and community discharge planning.

Additional Information:

  • Location: Lambeth Civic Centre (hybrid working model).
  • Full-time: 35 hours per week (Monday to Friday, 09:00 – 17:00).
  • Inside IR35.
