Job Description
Social Worker – Adults Hospital Pathway Team – London Borough of Sutton
The London Borough of Sutton is recruiting two experienced Adult Social Workers to join the Hospital Pathway Team. These roles will focus on facilitating safe discharges, carrying out assessments, and safeguarding interventions for patients at St Helier Hospital and within the wider community post-discharge.
Key Responsibilities:
Role 1 – St Helier Hospital:
- Facilitate timely discharges from hospital in partnership with the Continuing Healthcare (CHC) team.
- Complete Care Act assessments, MCA assessments, Best Interest decisions, and participate in multi-agency discharge planning.
- Manage complex cases and safeguarding concerns related to patients within the hospital setting.
- Conduct DST assessments post-discharge and contribute to ongoing support planning.
Role 2 – Civic Offices / Community (D2A):
- Carry out Care Act assessments post-discharge under the Discharge to Assess (D2A) model.
- Manage safeguarding enquiries and oversee team safeguarding cases.
- Complete MCA assessments, Best Interest decisions, and DST assessments post-discharge.
- Manage complex cases, including involvement in Court of Protection applications when required.
Essential Requirements:
- Qualified Social Worker (QSW) with Social Work England registration.
- Experience working within hospital discharge, safeguarding, or adult community social work settings.
- Strong knowledge of the Care Act, Mental Capacity Act (MCA), and safeguarding frameworks.
- Excellent assessment, decision-making, and multi-agency working skills.
Additional Information:
- Role 1 based at St Helier Hospital.
- Role 2 based at Civic Offices, Sutton.
- Full-time: 36 hours per week (Monday to Friday, 09:00 – 17:00).
- Inside IR35.
