Job Description
Social Worker – Hospital Assessment Team – Neath Port Talbot Council
An excellent opportunity has arisen for an agency Social Worker to join Neath Port Talbot Council’s dedicated and motivated Hospital Assessment Team. You will play a pivotal role in ensuring safe, person-centred, and timely discharges from hospital, working within a supportive multi-disciplinary environment.
Key Responsibilities:
- Undertake person-centred assessments to support safe discharge planning.
- Complete Mental Capacity Assessments and lead Best Interest Meetings.
- Collaborate with health board colleagues, therapy teams, providers, and families.
- Conduct Continuing Health Care Assessments and liaise with multi-agency partners.
- Promote positive outcomes and empower individuals in their care journey.
- Maintain accurate records in line with statutory frameworks.
About you:
- Qualified Social Worker with Social Work England registration.
- Minimum of 2 years’ experience in adult social care, ideally within a hospital discharge or integrated health setting.
- Knowledge of the Mental Capacity Act, Care Act, and Continuing Health Care processes.
- Excellent assessment and risk management skills.
- Strong multi-agency working and communication skills.
